Information contained in the OMSC® database is sensitive in nature. Unauthorized access, misuse, or disclosure of personal information contained in the OMSC® database will result in the revocation of access rights and may result in legal action.

I understand and agree that:

• I have read and will comply with the Ottawa Model for Smoking Cessation® Privacy Notice.
• I will not access any information other than that required to carry out my obligations in the Ottawa Model for Smoking Cessation®.
• I will keep my password secure and I will not share it with anyone.
• I will report any privacy breaches to my site’s Privacy Officer and email OMSCDatabase@ottawaheart.ca.
